Adrienne Kowcz, PharmD, CDCES

Pharmacy Consultant

Adrienne is the Pharmacy Consultant of Switchbridge, LLC.  She received her Doctor of Pharmacy degree in 2014 from Northeastern University and is also a Certified Diabetes Care and Education Specialist (CDCES).  She has worked at the University of Maryland Quality Care Network with the School of Pharmacy, as part of a care management team comprised of pharmacists, social workers, nurses and health coaches.  The team was dedicated to delivering quality and cost-effective care to its members through interdisciplinary teamwork.

Adrienne provided medication therapy management and disease state education to roughly 300 members of the University of Maryland Medical System Employee Health Network in the Copay Waiver Program for diabetes and hypertension.  Through this model of pharmacist involvement, patient outcomes were evaluated and the benefit of a pharmacist helping to manage these chronic conditions was identified. She also led a grant-funded COPD Telehealth program with the University of Maryland Eastern Shore practices, to improve adherence, patient education, and to optimize therapy in a cost-efficient manner.

Adrienne also initiates patients on Tandem Diabetes Care pump therapy and provides education to endocrinologists on insulin pump therapy.

Adrienne has a passion for chronic condition management and specifically diabetes, having type 1 diabetes herself since the age of nine (9).  She strives to educate and empower patients to be proactive in managing their chronic conditions and take initiative to care for themselves.  In her spare time, she enjoys playing tennis and training for marathons and half-marathons, attending many concerts, and traveling as often as possible.
